There are times when a patient no longer needs the acute care first received in the hospital–yet may benefit from further rehabilitation services before you return home. For those patients, we offer Swing Bed Services. After surgery and hospitalization, patients may move their status from an acute care hospital patient to a swing bed patient. Others move from another hospital to a facility with swing bed care, closer to home. The ultimate goal of swing bed care is to help the patient improve their physical functioning before returning home.
Swing bed services can help patients who need:
- Long-term IV antibiotics
- Rehab or skilled care after a recent hospitalization
- Rehabilitation after surgery
- Therapy after a stroke, accident, or injury
- Wound care

Qualifications for Swing Bed Services
Patients are evaluated and admitted for Swing Bed care based on their medical and physical needs. Once it is determined that Swing Bed care is appropriate, qualifications include (but not limited to):
- Documented need for further nursing and therapy services
- Patients who require long term IV therapy services
- Qualifying 3-day in patient hospital stay (for most insurance providers)
- Wound care

- 24/7 hospital-trained nursing staff
- Dietitian
- Ministerial services
- Personalized activities
- Pharmacist
- Physical, Occupational, Respiratory and Speech therapy
- Primary physician or hospitalist
Discharge Criteria
- Achieved maximum benefits of skilled nursing per therapy
- Changes in patient's needs or level of care
- Established safe transitional plan for home
- Required home medical equipment needs arranged
- Specific goals have been met
